New patient forms
Your schedule is busy! We understand, and we respect your time. Please print these new patient forms. Complete them at your convenience, with your medical records nearby. Be sure to note medications and nutritional supplements you take, and feel free to jot down questions for the dentist. Then, bring the forms along to save time at your initial visit. If you have any questions while filling out the forms, feel free to call (901) 472-7474 for assistance from our helpful front desk staff.
We believe that everyone has a right to oral health and a lovely smile. Please don’t postpone dental care because you feel you cannot afford it. Dental Care of Germantown will work with you to satisfy your dental needs affordably, regardless of insurance status or coverage allowance.
If you have insurance, we help you get maximum value from those benefits. We accept all plans, including TennCare, and are happy to obtain an estimate of coverage, and file claims on your behalf. However, co-pays are due in full at the time of service, and you are responsible for any non-covered portion of costs. Please bring your insurance card and plan information to your first visit and let us know any time you change plans or coverage.
For patients without insurance, or for out-of-pocket expenses, we:
- Accept cash, personal checks, and major debit/credit cards.
- Assist with application for CareCredit. Upon approval, this special healthcare credit card offers flexible low- and zero-interest financing options.
- Offer in-house financing (call or visit our front desk for details).
